6. 10+2 Marksheet (Scan Copy of Original)
7. Attested Copy of Degree Certificate OR Provisional Degree Certificate
8. Three Recent Passport Size Photographs (5cm × 4cm)
9. Affidavit (If Degree from Inside or Outside State)
   • As per specimen copy in website download section
   • For authenticity of submitted documents
10. Proof of Employment:
    • Employer Certificate mentioning:
      • Service Period
      • Designation
      • Head Quarter
      • Practitioner License No (If Available)
    • Self Employed Candidates:
      • Self Declaration on Letter Pad of Firm / Hospital
11. Affidavit (Judicial Magistrate First Class)
    • Required if already registered in other State Council
