All Marksheets of 8 Semesters of BAMS/BUMS.
Training Certificate (One month For Industrial Training or 150 hours For Hospital training).
3. Age Proof Certificate (Class X Admit Card/ Birth Certificate)
I.D. Proof: Voter I.D. Card/ Aadhaar Card/ Passport
Character Certificates (In Original)
10+2 Marksheet (Scan Copy of Original)
Attested Copy of Degree Certificate or provisional Degree certificate.
Three Recent Passport Size Photographs (5 X 4 Cms)
An affidavit (as per specimen copy in Website download section in case of applicant who has passed Degree in BAMS/ BUMS examination from an Institution within state and from other states for undertaking mentioning the authenticity of all documents submitted during New Registration.
10. Proof of Employment of the candidate: (a) Employer's Certificate mentioning period of service, designation, head quarters, etc. from relevant Firm/Company/Institution mentioning practitioner License No’s, if any ; or (b) In case of self employed persons(License): A self declaration on the letter pad of the relevant firm/Hospital etc.
